Your Pre-Registration Form
Before your appointment, please download, print and fill out your pre-registration form. This form must be completed before your mammogram and you’ll need to bring it to your appointment.
Tips for Your Appointment
To make your experience as comfortable as possible:
- Be prepared to provide the name, address and phone number of your current physician so that your results are sent to the appropriate place
- Do not wear any deodorant, lotion or creams around your chest area the day of your appointment
- On the day of your appointment, wear a two-piece outfit, so you only have to remove your top (you will receive a gown to wear)
Changing or Canceling Your Appointment
You will receive a confirmation email from MobileMammographyWFD@AdventHealth.com with your appointment date and time. If you do not see it in your inbox, please check your junk or spam folder. Follow the link in that email to change or cancel your appointment.
To change your appointment time, you must first click on the link in the email, which will cancel your appointment. Then, schedule a new appointment online.
To cancel your appointment, simply click the link in your confirmation email.
Using our website is the fastest way to schedule, change and cancel an appointment. However, if you need help, call us at 813-601-1925 and your call will be returned within 24 hours.

Our mobile mammography coach offers only screening mammograms (routine, annual exams for people with no current breast problems). If you are under the age of 40, you need a prescription from your physician for a screening mammogram. If you are over the age of 40, no prescription is required. If you currently have concerns regarding your breast health (e.g., a lump you can feel, nipple discharge, pain, dimpling of the breast or nipple changes), please contact your physician immediately.
